The camera is a dedicated black and white version of the GR IV, developed around a backlit APS C monochrome CMOS sensor with 25.74 megapixels without an anti-aliasing filter.

The sensor records brightness data directly in each pixel, and the camera is combined with an 18.3 mm f/2.8 GR lens. The model uses GR Engine 7 and offers ISO up to 409600. A built-in red filter can be activated via the Fn button for classic contrast effects. Two monochrome image controls are included: 'Solid' with a stronger tone curve and 'Grainy' with cinematic grain. GR IV Monochrome has a mechanical and electronic shutter, with the latter going up to 1/16000 s. The electronic shutter works from 1/3200 s at f/2.8 and from 1/5000 s at f/5.6-16. The model has 5-axis image stabilisation, 53 GB internal memory, and an 1800 mAh DB-120 battery with a capacity for around 250 images.
